Mills Gap pullout overlooks the James River Valley, which is 1,585 feet lower in elevation than the Blue Ridge Parkway. There is a decent scenic view here, but if you drive only .2 mile south to the next pullout, you get a much better look at the very same scenery. At Mills Gap there is a large tree in the center of the overlook, plus the overlook is on an outward curving bend in the Parkway (like being on the bottom of a U), which limits the scope of the vista.
The Appalachian Trail passes through the Mills Gap parking lot. The southern trailhead is on the other side of the Blue Ridge Parkway, and the northern trailhead is at the north end of the parking lot.
